Please call one of our friendly staff on (03) 477 6471 or email roslynhealth@xtra.co.nz to request an enrolment form or call into the clinic during opening hours to fill in a form and talk to one of our staff.

We require you to provide photographic identification and we may also require proof of your eligibility for government funded healthcare, usually by sighting your passport and / or visa.

Roslyn Health Centre is an active teaching practice. As part of your consult, you may be asked to allow a medical student to interview you or be present for your consult. You are under no obligation to see a student at any time.

Due to COVID-19, if you have flu like symptoms, fever or cough. Please call (03) 477 6471 to be assessed by a nurse.

  • Appointment times are approximately 15 minutes.
  • Appointments are one person per appointment.
  • You must be a registered patient at Roslyn Health Centre and registered to Manage My Health to book an appointment via Manage My Health.
  • Online appointments are for 15 minutes only. For a double appointment, please call our receptionist on (03) 477 6471.

Please check:

  • That you have been seen by your doctor within the last six months (or within a different time period agreed by your doctor)
  • That we have at least 24 hours (preferably 48 hours) notice to prepare the prescription for collection or to send it to your pharmacy.
